Separating climate resilient crops through screening of drought tolerant rice land races in Nepal

Abstract: Many studies have shown that local landraces are found to be better adapted to changing climatic conditions. The screening of local land races is one of important processes to detect drought tolerant behavior of crop species to further verify in similar field situation and genomic/molecular level studies. In 2009 for main season rice, a study was done to identify different land races of rice in Jhapa and Kailali districts to know their performance on drought stressed conditions by field techniques. Among the 22 rice land races studied, Kataush showed maximum drought tolerant efficiency (DTE) of 192.8% and minimum drought susceptibility index (DSI) of -0.88% followed by Guthanisaro (DTE=181% and DSI= -0.77%). Likewise, in the cluster analysis, these genotypes formed a cluster with two highest DTE and minimum DSI. Hence, these two land races could be used for drought stressed breeding program in the future to include drought resistant genes for variety impovement. Also, it was recorded that these land races could be used as the most drought tolerant variety in Terai region similar to Jhapa and Kailali, Nepal. These varieties could play important role in climate extreme situations, where farmers can adopt them as climatic resilient rice genotypes. Evaluation of a patient-initiated review system in rheumatoid arthritis: an implementation trial protocol

Abstract: Three hundred and eighty patients attending routine review at Plymouth Hospitals NHS Trust will be randomised to either enrol immediately into a patient-initiated review system (direct access group), or to be seen regularly by a clinician at the hospital (regular clinician-initiated group). Patients (or their general practitioner) in the direct access group can arrange a review by calling a rheumatology nurse-led advice line that enables telephone delivered clinical advice, or where appropriate, an appointment with a rheumatologist within 10 working days. Patients in the regular clinician-initiated group will attend their planned appointments at regular intervals during the intervening period of 12 months. The primary outcome of interest is patient satisfaction; secondary outcomes include service use, waiting times and clinical measures. Semi-structured, in-depth interviews will be conducted with a subset of patients and staff with the aim of identifying facilitators/barriers in implementing patient-initiated clinics.The implementation of a patient-initiated review system in routine care rheumatology will replace the fixed clinician-driven review system with a more flexible patient-driven system where patients usually self-manage their disease, but can request prompt help when required. We believe that this study will enable a comparison of the changes in local services and will be helpful in exploring the benefits/drawbacks of such implementation, thus providing lessons for implementation in other hospitals and for other chronic diseases.

A Diagnosis Missed for Several Years- Wegener’s Granulomatosis

Abstract: Wegener’s granulomatosis is a form of systemic vasculitis of small to medium sized vessels and affects upper respiratory tract, lungs and kidneys along with various organs. It causes necrotizing granulomatous inflammation of the affected parts and presents with positive antineutrophil cytoplasmic antibodies in more severe forms. Being a systemic disease with the potential to affect any organ-systems with a wide range of clinical presentations, it is associated with a risk of delay in diagnosis with resultant setback in institution of appropriate treatment. Confusion may arise due to an extent of histological similarity between Wegener’s granulomatosis and the more prevalent tuberculosis, both causing granulomatous inflammation of the affected parts. Here, we present two cases of this rare disorder where the diagnosis was missed for several years in the beginning causing a delay in institution of specific therapy which led to the development of complications. First Demonstration of Imaging Cosmic Muons in a Two-Phase Liquid Argon TPC using an EMCCD Camera and a THGEM

Abstract: Colossal two-phase Liquid Argon Time Projection Chambers (LAr TPCs) are a proposed option for future long-baseline neutrino experiments. This study illustrates the feasibility of using an EMCCD camera to capture light induced by single cosmic events in a two-phase LAr TPC employing a THGEM. An Andor iXon Ultra 897 EMCCD camera was externally mounted via a borosilicate glass viewport on the Liverpool two-phase LAr TPC. The camera successfully captured the secondary scintillation light produced at the THGEM holes that had been induced by cosmic events. The light collection capability of the camera for various EMCCD gains was assessed. For a THGEM gain of 64 and an EMCCD gain of 1000, clear images were captured with an average signal-to-noise ratio of 6. Preliminary 3D reconstruction of straight cosmic muon tracks has been performed by combining the camera images, PMT signals and THGEM charge data. Reconstructed cosmic muon tracks were used to determine THGEM gain and to calibrate the intensity levels of the EMCCD image.
An Adolescent with innumerable myomas

Abstract: Myoma is rare in younger age group, so tends to be mistaken for adnexal tumour. A 19 year old ?adolescent who was taken for laparotomy for ovarian germ cell tumour instead was found to have ?uterus studded with innumerable myomas, which were surgically removed through tedious yet ?successful myomectomy of 40 myomas, thus deserving description. Primary tongue tuberculosis: a case report

Abstract: Introduction: Tuberculosis of the tongue is one of the less common lesions caused by the mycobacterium bacilli whether primary or secondary. Most of the reported cases in the literature are in association with pulmonary lesion or a primary focus elsewhere. Case Report: We report a case of 63 years old male patient from Rajghat, Morang, Nepal with burning sensation over tongue since 30 days. Microscopic examination revealed epithelioid cell granuloma, caseous necrosis and Langhans giant cell with Acid Fast Bacilli on Ziehl Neelsen staining. Study on the prevalence of Cryptosporidium in calves and HIV infected humans in the periphery of river basins of Kathmandu valley

Abstract: I ntroduction : Cryptosporidium is an important zoonotic pathogen transmitted primarily through water. This study was conducted to determine the occurrence of Cryptosporidium in calves and humans in relation to the river water in the river basins of Kathmandu. M aterials and Methods : This cross sectional study was conducted from Sept 2012 to Feb 2013. A total of 100 fecal samples (50 calves and 50 HIV/AIDS patients living near the river basins of Kathmandu) were examined for the presence of Cryptosporidium by Ziehl-Neelsen Staining technique after Modified Sheather concentration method with centrifugation. R esults : Overall 32% calves and 18% humans were positive for Cryptosporidium oocysts, calves having higher chances of being infected than humans (P < 0.05). The prevalence was higher in Manohara River basin than Bagmati River basin (P >0.05). Among calves, the samples from Manohara periphery had higher prevalence than Bagmati periphery (P< 0.05). Likewise, the prevalence was higher in 4-6 months aged calves (P < 0.05) and in males (P < 0.05). Among the human samples, the females were at lower risk (odds ratio, 0.44) and 0-20 years of age group had highest prevalence (P < 0.05). Humans from periphery of Manohara were at higher risk than from Bagmati (Odds ratio, 4.94). C onclusions : Cryptosporidium has been identified in calves and HIV/AIDS patients residing in the periphery of Manohara and Bagmati river basins of Kathmandu valley suggesting the occurrence of zoonotic transmission. Cryptosporidiosis should be included in the differential diagnosis for the case of diarrhea in humans and domestic animals. Histoid leprosy on scrotal skin: an unusual case

Abstract: INTRODUCTION: Although no part of the skin is immune from invasion by mycobacterium leprae it is commonly seen over cooler parts of the body and very rarely found over external genitalia because of their warm temperature. Histoid leprosy is rare but a well-defined entity with specific clinical, histopathological, and bacteriological features. CASE REPORT: A Punch biopsy measuring 3x3 cm of a 73 year old male having an erythematous plaque over scrotum since 2 months was received for histopathologic examination with the clinical differentials of pilomatrixoma and steatocystoma multiplex. Histopathologic and special stain slides revealed a feature of histoid leprosy. CONCLUSION: Although rare, leprosy lesions may occur on the male genitalia and therefore in all male patients, history taking and examination of external genitalia should not be neglected. ? Kondo lattice and antiferromagnetic behavior in quaternary CeTAl$_4$Si$_2$ (T~=~Rh, Ir) single crystals

Abstract: We report the synthesis and the magnetic properties of single crystalline CeRhAl$_4$Si$_2$ and CeIrAl$_4$Si$_2$ and their non magnetic La-analogs. The single crystals of these quaternary compounds were grown using Al-Si binary eutectic as flux. The anisotropic magnetic properties of the cerium compounds were explored in detail by means of magnetic susceptibility, isothermal magnetization, electrical resistivity, magnetoresistivity and heat capacity measurements. Both CeRhAl$_4$Si$_2$ and CeIrAl$_4$Si$_2$ undergo two antiferromagnetic transitions, first from the paramagnetic to an antiferromagnetic state at $T_{\rm N1}$~=~12.6~K and 15.5~K, followed by a second transition at lower temperatures $T_{\rm N2}$~=~9.4~K and 13.8~K, respectively. The paramagnetic susceptibility is highly anisotropic and its temperature dependence in the magnetically ordered state suggests the $c$-axis to be the relatively easy axis of magnetization. Concomitantly, isothermal magnetization at 2~K along the $c$-axis shows a sharp spin-flop transition accompanied by a sizeable hysteresis, while it varies nearly linearly with field along the [100] direction up to the highest field 14~T, of our measurement. The electrical resistivity provides evidence of the Kondo interaction in both compounds, inferred from its $-lnT$ behavior in the paramagnetic region. The heat capacity data confirm the bulk nature of the two magnetic transitions in each compound, and further confirm the presence of Kondo interaction by a reduced value of the entropy associated with the magnetic ordering. From the heat capacity data below 1~K, the coefficient of the linear term in the electronic heat capacity, $\gamma$, is inferred to be 195.6 and 49.4~mJ/mol K$^2$ in CeRhAl$_4$Si$_2$ and CeIrAl$_4$Si$_2$, respectively classifying these materials as moderate heavy fermion compounds.
